ClickUp Forms: Building Forms That Turn Answers Into Tasks

ClickUp Forms collect answers and turn every submission into a task in the List you chose when you built the form, with each answer landing in a task field or Custom Field. Forms are on all plans, but as of October 2026 a Free Forever Workspace can only create one Form.

The bigger catch sits in the features people want most. Conditional logic, hidden fields and Form options start on Business Plus, and nesting logic more than one level deep needs Enterprise or Max. Unlimited and Core can share and embed forms but get 5 exports. And when you do export responses to CSV, ClickUp only includes standard fields: Custom Fields aren't in the export.

Every answer becomes part of a task

This is what makes ClickUp Forms different from a standalone form builder. Each question is built on a field. Map a question to a task field like Due Date, Priority, Assignee or Task Description and the answer fills that field. Map it to a new or existing Custom Field and it fills that instead, and new Custom Fields are added to the Form's location so you can use them on other tasks there. Private Custom Fields work too.

A Form can only use Custom Fields that exist in the Hierarchy where it was created. A Form made from a List can use fields from that List, its Folder and its Space. A Form made from a Space can use fields from the submission List, the Space and the Workspace. If you don't add a Task Name question, each submission task is called Form Submission followed by a timestamp, so add one if you want task names your team can scan.

Two limits apply on every plan. Each uploaded file is capped at 25 MB, whether it goes to the task's Attachments or a Files Custom Field, and that's separate from the limit for files attached directly to tasks. And a form accepts 500 responses per hour from any one IP address. Account Authenticated Forms, which only let people in your Workspace view and submit, are Enterprise and Max only.

Building a form step by step

  1. In the Views Bar, select Form. You can also start from Forms Hub, or hover over a Space, Folder, Subfolder or List and click the plus icon.
  2. Pick a template or Start from scratch, and select the List where submissions will be saved.
  3. In the Build tab, click Add question, then search for a field or choose a question type.
  4. Choose Create new field to make a new Custom Field, or Map to for an existing one.
  5. Edit the question name and description, tick Required if it must be answered, then click out of the question to save it.
  6. Open the Settings tab to choose a layout and colours, apply a task template or auto-assign submission tasks.
  7. Check the Published status in the upper-right corner, then click Copy link.

Renaming a Custom Field on a Form only changes the label on the Form; everywhere else in the Workspace it keeps its original name. If a Custom Field has a default value, the question pre-fills with it.

Conditional logic and the delete trap

Logic works on Assignee, Checkbox, Dropdown, Label, People, Priority and Status questions. Turn on Conditional Logic in the lower-right corner of the question, click Add rule and set the condition. You can add up to 25 rules per question. ClickUp's example adds a Due Date field to the task when someone picks Urgent priority in a dropdown.

Be careful when you tidy up. Deleting a question deletes every question nested below it. Switching a question's Conditional Logic toggle off doesn't delete the question itself, but it removes its rules and every question nested under them. With the One per page layout, each revealed question gets its own page straight after the one that triggered it.

Design, hidden fields and sharing

There are three layouts: One column, Two column, and One per page, which shows a welcome screen and then one question at a time. The builder shows every question together, so open the public Form URL to see the real flow. ClickUp recommends cover images wider than 1480 pixels, and the Preview tab doesn't show the cover. Hiding ClickUp branding and adding your own logo needs Business Plus or Enterprise.

Hidden fields let you pass answers you already know, like where a lead came from, through the Form URL instead of asking. You can hide Email, Text, URL, Dropdown, Priority and Number Custom Fields. Field labels and values are case-sensitive, and special characters aren't supported.

In the sharing settings you can make the link expire, let search engines index it, which is off by default, and copy the embed code. Forms are published by default, so check the status before you send the link anywhere.

Work with me

Want AI doing the heavy lifting in your marketing?

I build the systems that handle the boring 80 percent, so you get your week back. Done properly, with the human kept in.

Where submissions end up

Submissions exist in ClickUp only as the tasks they create. Click the responses button to jump to the List, or the download button next to it to export everything as CSV. ClickUp notes that if a submission fails to create a task, it won't show in your Workspace Audit Log, so you'd need to contact ClickUp Support. Once tasks are flowing in, you can ask Brain AI about them from the location header or chart them on a Dashboard.

My verdict on ClickUp Forms

I'd use ClickUp Forms for intake that should turn straight into work, like project requests, IT tickets or client briefs, because every answer lands in a field your team already works from. I'd skip them for anything that needs branching questions unless you're on Business Plus, and on Free Forever you only get one form. If your form is really for marketing leads, my guide to HubSpot forms is the better fit, and my ClickUp review covers the rest of the tool.

Intake forms are one of the easiest places to take manual copying and pasting out of a team's week. I run AI training and workshops for teams, and help businesses with AI enablement and workflow automation, so the busywork runs itself and people get time back. Most of that work is with SaaS marketing teams and small businesses. Here's how we can work together.
