Notion Forms: How to Build One From a Database and Share It

Notion forms collect answers from anyone, even people who aren't in your workspace or don't use Notion, and every question is tied to a property in a database, so each response lands as a page you can sort and filter. You start one with /form on a page or by adding a Form view to an existing database, and as of October 2026 all members on all Notion plans can create and use them.

Here's the catch. By default, editing a question in the form renames the database property behind it. Change "Favorite color" to "What's your favorite color?" and the database property is renamed to match, so the column you've been sorting by gets a new name. Switch off Sync with property name on that question if you want the form wording and the property name to differ.

Two more things trip people up. You can only create or customise a form in Notion's desktop and web apps, not on a phone. And conditional logic, where the next question depends on the last answer, is only on the Business and Enterprise Plans.

Two ways to start a Notion form

Starting pointHow it works, as of October 2026
A new formType /form on a page and select Form. A Respondent property is created automatically to capture names
An existing databaseSelect + next to your views, or Add view, then Form, then Create questions. You need Full access to the database
Capturing names on an existing databaseAdd a Created by property yourself, because it isn't created for you

The existing database route is the one I'd use most. If you've already been collecting information from people in a database, adding a form view means new answers drop straight in next to the old ones. If you haven't built the database yet, here's how to create a database in Notion first.

Setting up questions in the form builder

  1. Give the form a title and an optional description, then hover over the title to add an icon or cover.
  2. Click into each question to edit it, and select + under the last question to add more.
  3. Hover over a question and select the three dots to open its settings.
  4. Open Form settings, then Submit screen, to change the submit button and the confirmation message.
  5. Select Preview at the top to see the form as respondents will.
Question settingWhat it does
RequiredRespondents have to answer it to submit the form
DescriptionAdds a line explaining what you're asking
List view or dropdownChanges how multiple choice options are shown
Long answerGives text questions a bigger answer box
Question typeThe same as the property type, such as Multiple choice or Date
Maximum selectionsCaps answers on multi-select, relation and people questions
Sync with property nameOn by default; turn it off to word the question differently from the property
Conditional logicMultiple choice only, Business and Enterprise Plans only

The submit screen lets you change the button's colour and text, plus the confirmation title and body people see after they send. You can also choose to receive an email copy of each submission, which is handy if you don't live inside Notion all day.

Who can fill out your form

Select Share form at the top of the builder, then Who can fill out. There are three options.

OptionWho can respond
Anyone at your workspace with linkWorkspace members with the link. Guests only if they've been given access to the database behind the form
Anyone on the web with linkAnyone with the link, including people who don't use Notion. Responses are automatically anonymous
No accessNobody. Use it to close the form and stop new responses

If the form is workspace only and anonymous responses are off, Access to submission decides what respondents can do with their own answers afterwards: No access, Can view, Can comment, Can edit, or Full access, which also lets them change permissions on the page holding their response. On a paid plan, a web form can also have its Notion branding switched off.

One warning from Notion's help that's easy to miss. If a form came with a template you duplicated, set up a fresh form from scratch, so the responses come to you and not to the person who made the original.

The access levels behind each task

To do thisYou need on the connected database
Create a form from an existing databaseFull access
Edit a formCan edit or Full access
Change sharing settingsFull access
Copy and share the form linkAt least View access
View responsesAt least Can view
Edit responsesAt least Can edit content

Where Notion form responses end up

Responses are stored in a Table view of your database called Responses, with each question showing as a property. You can filter and sort them, or add a chart view to see the answers visually. You can't export a Form view, so export from the Table view instead. Notion also notes that a form with a very high volume of responses can be slower to load, and suggests creating a second version or deleting older responses you no longer need.

Respondents can fill a form out on desktop or mobile as long as they have the link and the right permissions. Some forms only work when the person is logged into the workspace that made them, and anyone who wants a copy of their own response has to log into Notion or create an account.

Automations and locking forms down

Forms can trigger automations, such as sending a status update to the respondent or a Slack notification when a new response comes in or a question is answered a certain way. Open the form builder, select the three dots at the top, then Automations, and add your triggers and actions.

Work with me

Want AI doing the heavy lifting in your marketing?

I build the systems that handle the boring 80 percent, so you get your week back. Done properly, with the human kept in.

On the Enterprise Plan, workspace owners can stop members sharing forms to the web from Settings, then Security, by turning on Disable publishing sites, forms, and public links. Any forms already live on the web come down straight away, and the same switch stops members publishing Notion Sites too.

My verdict on Notion forms

I'd use Notion forms whenever the answers need to end up in a Notion database anyway, because they skip the copy and paste step completely. Turn off Sync with property name before you start rewording questions, and check the access table before you share the link. If you need branching questions on a cheaper plan, you won't get them here, so it's worth seeing what Notion's free plan includes before you build around it.

If you'd like help turning forms, databases and automations into a workflow your team uses every day, that's the kind of thing I do. I run AI training and workshops for teams, and help businesses with AI enablement, AI automation and GEO, which is about getting your business seen and cited in AI answers. Most of that work is with SaaS marketing teams and small businesses. Here's how we can work together.
