How to Become a Paramedic: US and UK Routes Compared

To become a paramedic in the US, you first train and work as an EMT, then complete a paramedic program and get licensed by your state. In the UK, you need a paramedic science degree approved by the Health and Care Professions Council (HCPC), or a paramedic degree apprenticeship, and then you register with the HCPC.

The two countries treat the job very differently. In the US it's a step up a ladder that starts with EMT training. In the UK it's a degree-level profession from the start.

US facts come from the Bureau of Labor Statistics (BLS) and the National Registry of Emergency Medical Technicians (NREMT); UK facts come from the National Careers Service. All were read on 26 October 2026. This is a careers guide, not medical advice.

US vs UK paramedic routes side by side

United StatesUnited Kingdom (England)
Starting pointEMT training first; paramedic programs require EMT-Intermediate certificationA paramedic science degree, usually 3 years full time, or a degree apprenticeship
TrainingA paramedic program, leading to a nondegree award, an associate's or a bachelor's degreeHCPC-approved degree, or a 3-year Paramedic Level 6 Degree Apprenticeship
RegulationState licence; most states require certification, others an equivalent state examRegistration with the Health and Care Professions Council
DrivingEmployers typically require a driver's licenceMany ambulance services expect a full licence, ideally C1, by the end of training

The US route, step by step

  1. Get a high school diploma and CPR and basic life support certification. BLS says programs usually ask for these.
  2. Complete EMT-Basic training. It typically takes less than 1 year.
  3. Move up to EMT-Intermediate. Programs can take up to 2 years, and paramedic programs require this certification.
  4. Gain work experience as an EMT. BLS says paramedics typically need it before a paramedic program, and volunteer work can help.
  5. Complete a paramedic program. Expect anatomy and physiology, EKG interpretation and supervised field experience.
  6. Get certified and licensed. Most states require certification, and NREMT offers paramedic certification pathways for people finishing a program, those with a current state licence, and those returning after a lapse.

The thing most people don't realise

In the US, you can't go straight to paramedic. BLS is clear that paramedic programs require EMT-Intermediate certification, and that paramedics typically need EMT work experience first. So the real timeline includes time on an ambulance as an EMT, not just classroom hours.

The pay difference shows why the climb is worth it. BLS puts the median for paramedics at $60,600 in May 2025, against $44,470 for EMTs, as of October 2026. That's a big jump at the median for moving up a level.

Choosing your training provider

In the US, programs are offered by technical institutes, community colleges, universities and specialist emergency care training centres, and some teaching happens in a hospital or on an ambulance. Ask whether the course leads to a nondegree award, an associate's or a bachelor's, because that affects how easily you can move on later. BLS notes that paramedic-to-RN programs offer an accelerated route into registered nursing, and a degree can make that step smoother.

In the UK, the key check is simpler: the course must be approved by the HCPC, or you won't be able to register when you finish.

US pay and jobs

MeasureBLS figure
Median pay, paramedics (May 2025)$60,600 a year
Median pay, EMTs$44,470 a year
Median pay, EMTs and paramedics combined$48,150 a year, $23.15 an hour
Projected growth 2025 to 20356%, faster than average
Openings each yearAbout 18,200 on average

Paramedics in local government had the highest median of the top settings, at $61,980. Note that BLS's employment data excludes volunteer EMTs and paramedics.

The UK route, step by step

  1. Get the entry qualifications. For a degree you'll usually need 2 or 3 A levels, including a science.
  2. Choose your route: a paramedic science degree approved by the HCPC, or a Paramedic Level 6 Degree Apprenticeship that mixes on-the-job training and classroom learning.
  3. Or work your way in. You could start as an ambulance care assistant or an emergency medical technician, also called an associate ambulance practitioner, and later apply for a trainee paramedic scheme with an ambulance service trust.
  4. Sort your driving licence. Many ambulance services expect a full licence by the end of training, preferably C1 for medium-sized vehicles.
  5. Register with the HCPC before you practise.

Entry to UK paramedic courses is very competitive, the National Careers Service says, so relevant volunteering helps, such as first responder work with a charity or an NHS ambulance trust.

What the job asks of you

  • Physical fitness: lifting, kneeling and bending are constant.
  • Long shifts: some US EMTs and paramedics work 12, 18 or 24-hour shifts.
  • Calm under pressure, because life-or-death calls are part of the work.
  • Working outdoors in all weather.

My verdict

In the US, think of paramedic as the top of a ladder: EMT-Basic, EMT-Intermediate, real experience on the road, then the paramedic program. It's worth the climb, with a median of $60,600 against $44,470 for EMTs. In the UK, the decision is degree or apprenticeship, and both end with HCPC registration. Whichever country you're in, sort the driving licence early, because it can hold up an otherwise ready candidate.
