Where to Get Free Fax Cover Sheet Templates (2026): Printable, Word and PDF

Vertex42 is the best free source for a plain fax cover sheet: it gives you a printable PDF, a Word file and an Excel or Google Sheets version, all free to use in your own office. Template.net has the widest choice of themed cover sheets in Word and PDF, and if you fax online, GotFreeFax and FaxZero will add a cover page for you.

If you run a small business that deals with clinics, insurers, law firms or government offices, you'll still get asked to fax something now and then. Template.net puts it plainly: fax cover sheets are used by people in healthcare, legal, business and admin roles who rely on fax. The cover sheet is the bit that tells the other office who it's from, who it's for and how many pages should have arrived. Get it wrong and your papers sit in a tray with nobody's name on them.

Those limits are what each site showed as of October 2026. Free fax services change their rules, so check the send page before you rely on them for something urgent.

Vertex42: made for a busy front desk

Vertex42 says what most offices do: they make one blank cover sheet, print a stack of copies and leave them by the fax machine. So its sheets leave plenty of room to write a quick message by hand. You get two printable PDFs. The Basic Fax Cover Sheet is fully blank, and the Company Fax Cover Sheet adds a space for your company name. Both come as Word files too, and the newer versions have editable fields.

There's also a version for Excel and Google Sheets, which Vertex42 says was one of the first fax templates built for a spreadsheet, plus an OpenOffice copy. It works in Excel 2007 or later and in Excel for iPad and iPhone. That's handy if you'd rather type the details once, save the file with your logo and print from there.

One small detail on both PDFs is worth copying: they make it clear the number of pages includes the cover sheet as page 1. That's the line people get wrong most often. If you're sending a 3-page contract, the box says 4.

What the Vertex42 licence lets you do

The licence is called Private Use. In plain words, you can download it, customise it and use it yourself or inside your company. You can't remove the Vertex42 logo or notices, and you can't sell it, put it in an online gallery or host it on your website for other people to download. Sharing an edited copy privately with your own team is fine. If you keep it in Google Drive, Vertex42 says link sharing must be off, so no 'anyone with the link'. Fine for your office, not fine as a freebie on your site.

Template.net: a cover sheet for every kind of business

Template.net is where to look if you want the cover sheet to look like it belongs to your business. Its list runs from Finance Fax Cover Sheet, Legal, Medical, Hospital and Real Estate covers to a Confidential Fax Cover, a Resume Fax Cover and a CV version. There are fun ones too: Bakery, Handyman, Gym, Church and Pub fax paper templates, and a Startup one.

You can edit each one in the browser and download it in Word, Google Docs, PDF or Apple Pages, and there's an AI Fax Sheet Generator if you'd rather describe what you want. Template.net's own advice for filling one in is sensible: accurate sender and recipient details, the date, the total pages, a short note on what's inside, and a clear mark if the fax is confidential or urgent.

When the online fax service makes the cover for you

If you don't own a fax machine, you might not need a template at all. GotFreeFax lets you choose a cover page as you send, and says it's free, has no ads and doesn't count toward your page limit. Its free service allows 3 pages per fax and 2 free faxes a day, and you can upload PDF, Doc or JPG files up to 40 MB in total.

FaxZero lets you type text to appear on its cover page, and you can send just an attachment, just text, or both. The free version puts FaxZero branding on the cover page, caps you at 3 pages plus the cover and allows 5 free faxes a day. Its paid option drops the branding or the cover page altogether.

Work with me

Want AI doing the heavy lifting in your marketing?

I build the systems that handle the boring 80 percent, so you get your week back. Done properly, with the human kept in.

Here's the small-business catch. A branded cover from a free service is fine for a quick form to a supplier. For anything confidential, like a client file from a law office or a patient letter from a clinic, I'd make your own cover the first page of the PDF so it says exactly what you want. Just remember that page then counts toward the free page limit.

Filling in a cover sheet so nothing goes astray

  1. Pick the format: a blank PDF if you write by hand, or the Word or Excel version if you'd rather type.
  2. Add your company name and fax number once, then save that as your master copy.
  3. Fill in the sender and recipient names, their fax numbers, the date and a subject line.
  4. Count every page you're sending and add one for the cover, because the cover is page 1.
  5. Write one or two lines on what's attached, and mark it confidential or urgent when it needs to be.
  6. Print a few blanks for the fax machine, or save the filled-in PDF to attach when you fax online.

Not sure how you'll send it yet? My guide to sending a fax from email covers the address format, there's a walkthrough for faxing from an iPhone, and if you fax often, see my online fax services compared.

My take on fax cover sheets

I'd grab the Vertex42 Basic PDF if you just need something to print and keep by the machine. It's clean, it's free and it gets the page count right. I'd pick a Template.net sheet if your faxes go to clients and you want the cover to match your brand. Skip the designed templates if you only ever fax through FaxZero's free tier, because it adds its own branded cover anyway, and skip any free branded cover when the fax is confidential.

Frequently asked questions

Where can I get a free fax cover sheet template?

Vertex42 has free printable fax cover sheets as a PDF, a Word file and an Excel or Google Sheets template. Template.net has themed cover sheets in Word, Google Docs, PDF and Apple Pages.

Does the fax cover sheet count as a page?

Yes on a normal fax. Vertex42's sheets say the number of pages includes the cover sheet as page 1. GotFreeFax is an exception: its own free cover page doesn't count toward its page limit.

Can I use a Vertex42 fax template for my business?

Yes, inside your own company. Its Private Use licence lets you customise and use it there, but you can't sell it, host it on a website or remove the Vertex42 notices.

Does FaxZero add a cover page?

Yes. On the free service FaxZero puts its own branding on the cover page, and you can type text to appear on it. Free faxes are limited to 3 pages plus the cover.

What should a fax cover sheet include?

Sender and recipient names and fax numbers, the date, the total number of pages including the cover, a short note on what's attached, and a clear mark if it's confidential or urgent.
