Form 3911, Taxpayer Statement Regarding Refund, is the form you send the IRS to start a trace on a refund it says it issued but you never got, or a refund check that was lost, stolen or destroyed. Joint filers usually need it, because the online and phone trace is only for single, married filing separately and head of household returns. You send one form per refund.
This guide follows the October 2022 revision of Form 3911, the IRS About Form 3911 page and the IRS refunds page, read as of October 2026.
Wait for the full processing time first
A trace only makes sense once the IRS has issued the refund. The IRS says you should get a refund check within 4 weeks from the date it's issued, and you can see that date in the refund tracker or your online account. The tracker shows a status 24 hours after you e-file a current year return, 3 days after you e-file a prior year return and 4 weeks after you file on paper.
If the tracker doesn't show the refund as sent yet, Form 3911 won't speed anything up. It's for refunds already on their way, not for returns still being processed.
Which route your filing status puts you on
| Filing status | How to start a trace |
|---|---|
| Single, married filing separately, head of household | The refund tracker or the IRS app (current year refunds only), or the automated line on 800-829-1954 |
| Married filing jointly | Call the IRS on 800-829-1040, or complete and send Form 3911 |
| Business returns | Form 3911, sent to the Cincinnati or Ogden refund inquiry unit |
Joint returns are the main reason Form 3911 exists. Both spouses have to sign before the IRS can begin a trace, so the paper form collects both signatures in one go.
What the form asks for
- Section I: your name, taxpayer identification number and current address, and your spouse's if you filed jointly. Line 4 is the name and address exactly as they appeared on the return.
- Lines 5 and 6: a representative's name and address, if you authorized one to receive the check.
- Line 7: the type of return, the tax period, the date filed, whether you asked for a check or direct deposit, the refund amount and your bank details.
- Section II: whether you never received the refund, or received a check that was lost, stolen or destroyed, or received it and signed it.
- Section III: your signature under penalties of perjury, and your spouse's on a joint return.
If parts of the form don't apply, the instructions say you can mark them N/A. You're also asking the IRS to send a replacement, and you promise to return one if two arrive.
The endorsed check rule
Line 9 matters more than it looks. If you signed the check and someone else cashed it, the IRS can't issue a replacement, because that person didn't forge your signature. The same goes for an old check: one that wasn't cashed within a year of its issue date can't be cashed at all, so contact the IRS for how to return it.
Before you fill it in
Have your return to hand, because line 4 wants the name and address exactly as you wrote them there, and line 7 wants the tax period, filing date and refund amount. Give a phone number where you can be reached between 8 a.m. and 4 p.m. The IRS refund tracker shows the current processing year plus two prior years, so check the issue date there before you start. It doesn't track refunds claimed on an amended return, so a refund from Form 1040-X has its own amended return status tool.
You don't strictly have to use the form. The privacy notice says you can give the IRS the same information in a letter, though without it the IRS may be unable to trace or replace the refund. The form is simply the quickest way to make sure nothing's missed.
What happens after you send it
The outcome depends on what happened to the check. If it wasn't cashed and it's less than a year old, the IRS cancels it and mails a new one, which should arrive within 4 weeks of issue. If it was cashed, the Bureau of the Fiscal Service mails you a claim package with a copy of the cashed check. It compares the signature, and that review can take up to 6 weeks.

Where to send Form 3911
Mail or fax it to the refund inquiry unit for your state, not the address on a tax return. The IRS lists units in Andover, Atlanta, Austin, Brookhaven, Fresno, Kansas City, Memphis and Philadelphia, each with its own fax number. Taxpayers abroad, in a US territory or with an APO or FPO address go to Austin. Only send Form 3911 to those fax numbers, nothing else.
If a replacement refund comes up short because a debt was taken from it, that's a different problem. A joint filer whose share went to a spouse's debt uses Form 8379 instead.
The verdict
Check the refund tracker, wait out the 4 weeks from the issue date, and use the online trace if you can. If you filed jointly or you're a business, fill in Form 3911 with both signatures and fax it to your state's unit. Answer line 9 honestly, because an endorsed check changes the outcome.
