Physical therapy software combines clinical notes, scheduling, billing and patient communication, and some tools add home exercise programmes and telehealth. The right fit depends on your country, practice size and how you bill. Ten tools follow in no particular order.
Therapists didn't train for years to spend evenings writing notes. Yet documentation, scheduling and billing take a big share of every week, and clunky software makes it worse. The right system gets you back to patients and keeps your records in order.
The ten tools below are listed alphabetically and in no particular order, from practice management systems popular with UK and Australian physios to US PT EMRs with billing and AI scribes. Each description comes from the vendor's own site, re-checked this week. For ideas on using AI in a clinic, see my guide to AI for physiotherapy clinics.
Physical therapy software compared
In the free plan column, Yes means the tool's own site describes free use or a free plan, Trial only means a time-limited trial and No means neither is stated on the pages we read. This page is general information about software, not clinical or legal advice.

Ten physical therapy software tools
Cliniko
Best for: Physio and allied health clinics that want clear notes, booking and invoicing with a 30 day trial first.
Jane
Best for: Clinics with physios, massage therapists and other practitioners who want one booking and charting system.
Net Health
Best for: Larger rehab organisations that want specialty EHR tools shared across outpatient clinics and hospital-based therapy teams.
Physitrack
Best for: Therapists who want patients to follow exercise videos at home and track adherence between visits.
Prompt Health
Best for: Growing outpatient practices that want EMR, scheduling and billing automation built for rehab therapy.
PtEverywhere
Best for: Small PT clinics that want an AI scribe and assistant included in the plan rather than as an add-on.
Raintree
Best for: Fast-growing therapy groups with several sites that want EMR and revenue cycle tools in one platform.
SimplePractice
Best for: Solo therapists and small practices that want booking, notes and a client portal in a well-known EHR.
TheraOffice
Best for: Clinics that want AI documentation that filters background noise so notes are written while treating.
WriteUpp
Best for: UK private physios who want notes, diary and invoicing in a system built with UK clinicians in mind.
Direct access, and why your software should track visits
APTA says all 50 states, the District of Columbia and the U.S. Virgin Islands now have either provisional or unrestricted direct access to physical therapist services.
What that means for you: provisional access often comes with conditions before a referral is needed, which vary by state. Choose software that can track visits and dates per plan of care and remind you when a referral or update is due.
Want AI doing the heavy lifting in your marketing?
I build the systems that handle the boring 80 percent, so you get your week back. Done properly, with the human kept in.
Practice management or full PT EMR?
Practice management systems focus on booking, notes, invoicing and patient communication, which suits private clinics in the UK, Australia and elsewhere. Full US PT EMRs add insurance billing, compliance checks and outcomes reporting. Home exercise tools sit alongside either, giving patients videos and tracking between visits.
Ask each vendor about notes first. If writing a daily note is slow, nothing else in the system will make up for it.
Moving a clinic to new PT software
- Export patient records, notes and future appointments from the old system.
- Check how historical notes will be stored and searched.
- Set up note templates for your most common conditions.
- Build your home exercise library before go-live.
- Train front desk staff on booking and payments first.
- Review a week of notes and claims after the switch.
Features worth testing in a demo
- Daily note speed. Time yourself writing a typical note.
- Templates. Can you build your own for common conditions?
- Patient reminders. Check text and email options.
- Reporting. Can you see cancellations, visits and outcomes easily?
Using AI notes with care
Several systems now offer AI scribes that draft notes from the session. They can save real time, but you're still responsible for what's in the record. Read every draft before you sign it, check how audio's stored and make sure patients know when AI's helping with documentation. It's your name on the note, so don't let speed win over accuracy.
Keeping patients coming back
Many patients drop off before they finish their plan of care. Reminders, easy rescheduling and a home exercise programme they can follow on their phone all help. Track who misses sessions and reach out early, because a quick check-in often brings someone back on track.
Questions your team will ask
Your front desk will want to know how booking and payments work, and your clinicians will want to know about notes and templates. Don't leave those questions until go-live. Run a short session for each group, let them practise on dummy patients and you'll avoid most of the first-week frustration that makes people distrust new software. If something isn't working, they'll tell you quickly, so listen and adjust the templates rather than insisting everyone works around them.
Do you make physical therapy software? Tool makers can ask to be considered for a featured place on this list.
My plain advice on physical therapy software
Choose the system that makes documentation fast and accurate. Time saved on every note adds up to hours each week, and better notes protect you and your patients.
Then think about the patient side. Online booking, reminders and home exercise programmes keep people engaged between visits, which is good for outcomes and for your schedule. For ways to bring in new patients, see my guide to digital marketing for small clinics.
Frequently asked questions
What is physical therapy software?
It's software for clinical notes, scheduling, billing and patient communication, often with home exercise and telehealth tools.
Is there free physical therapy software?
None of the tools on this list describes a free plan on the pages we read, but several offer free trials.
Can patients see a physical therapist without a referral in the US?
APTA says all states now have provisional or unrestricted direct access, though conditions vary.
What is a PT EMR?
It's an electronic medical record built for physical therapy, with notes, billing and compliance tools for rehab practices.
Do physio clinics in the UK use the same software as US clinics?
Not always. UK clinics often prefer practice management systems without US insurance billing.
Can PT software include home exercise programmes?
Some do, and others connect to home exercise apps that send videos and track adherence.
