A HIPAA compliant texting app for healthcare is one of three things: a two-way texting tool for a practice, a patient engagement system tied to your EHR or practice management software, or secure messaging for a hospital. They're different purchases, so start with your setting, then read each vendor's own HIPAA wording.
Texting patients sounds like a single feature. Choosing a tool is harder than it looks, because texting apps, patient engagement suites and hospital messaging systems all get filed under the same search.
The ten tools below are listed alphabetically and in no particular order. Every line comes from each vendor's own website, read on 2 Nov 2026 (as of Nov 2026), and every HIPAA statement is the vendor's own wording, attributed to the vendor. I've skipped every read-rate statistic.

Three purchases that all get called patient texting
Here's what's easy to miss: a texting tool for a practice, a patient engagement tool tied to your EHR and a hospital messaging system are three different purchases. Sort the list that way first.
- Texting tools for practices (five). Two-way texting with patients, from tools built to end phone tag to a professional phone line with voicemail transcription. Two of the five are business texting platforms that also serve healthcare.
- Patient engagement tied to your systems (two). Texting wrapped into reminders, recall, telehealth, fax and portals that connect to practice software.
- Hospital and enterprise communication (two). Clinical messaging, on-call scheduling, contact centers and telehealth video.
- A hosting vendor's add-on (one). HIPAA Vault is a hosting company whose menu lists a texting product beside fax, email and forms.
How each vendor words its HIPAA claim
Read the exact words, because they differ. OhMD calls its product HIPAA compliant texting. Curogram offers 2-way HIPAA compliant texting. Spruce Health says it's designed for easy HIPAA compliance. SendHub says it's HIPAA-ready. Spok's secure messaging is there to help maintain HIPAA-compliant communications. RingCentral describes HIPAA-secure video consultations, and SimpleTexting, a business texting platform, publishes a guide to HIPAA compliant texting.
Want AI doing the heavy lifting in your marketing?
I build the systems that handle the boring 80 percent, so you get your week back. Done properly, with the human kept in.
Solutionreach talks about HIPAA-compliant automated communications, and Updox lists secure text and Direct secure messaging. Treat each line as the vendor's own description, and ask for the detail behind it in writing.
Before the first text goes to a patient
- Is this for a single practice, a group tied to an EHR, or a hospital?
- Does it connect to your practice management or EHR system?
- Do you need reminders, forms or text-to-pay, or just two-way conversation?
- Will staff text from a practice number rather than their personal phones?
- Which number types do you need: local, toll-free or short code?
- What does the vendor's HIPAA page say, in its own words?

My advice for practices that want to text patients
Match the tool to your setting before you compare features. A small practice that wants to stop phone tag needs a two-way texting tool, and a practice that lives in its EHR is better served by an engagement system that connects to it. Hospitals are buying clinical messaging, which is a different conversation. Then read each vendor's HIPAA wording word for word: HIPAA-ready, designed for easy HIPAA compliance and HIPAA compliant texting aren't the same sentence, so ask what's behind each one.
And if you run a dental practice, my guide on how dental practices can use AI to get more patients pairs well with this.
