Occupational therapy software helps OTs and practice owners run their work: scheduling, documentation, billing, client portals and, increasingly, AI note writing and home exercise programmes. Some tools are built for rehab clinics with PT and speech teams, others suit a solo OT. Ten options follow, in no particular order.
Occupational therapists spend a lot of time on paperwork. Assessments, goals, progress notes and reports all have to be complete, dated and clear, often for funders as well as clients, and the hours add up fast at the end of a clinic day.
The tools below are listed alphabetically and in no particular order, and each description comes from the provider's own website or app store listing, read this week. For the business side of a growing clinic, my guide to ways to improve your occupational therapy practice is a good companion.
Occupational therapy software at a glance
I checked the free plan column on each provider's own website or app store listing, as of Oct 2026. Yes means the page names the app, an account, a plan or a version of it as free. Trial only means a time-limited trial. Not stated means the page I read named neither, even where there's a button inviting you to start or a store download button, so check the provider's own page before you sign up.

Registration and record keeping
The Health and Care Professions Council (HCPC) lists occupational therapist as a protected title in the UK, so anyone using it must be registered with the HCPC. Registration comes with standards, including keeping full and accurate records.
Software can't meet those standards for you, but it can make good records the easy option: templates that prompt for the right details, automatic dates and signatures, and secure storage you can export if you ever change systems.
Practice systems, AI scribes and home programmes
Practice systems handle scheduling, documentation and billing in one place, and many are shared with physiotherapy and speech teams. They're the core of most clinics.
Want AI doing the heavy lifting in your marketing?
I build the systems that handle the boring 80 percent, so you get your week back. Done properly, with the human kept in.
AI scribes listen to a session and draft the note, which can save a lot of evening paperwork, though you'll still need to check every note. Home programme tools give clients exercises to follow between sessions and help you see whether they're doing them.
What to check before you buy
- OT templates. Look for assessment and goal templates built for occupational therapy.
- Data storage. Ask where records are held and how backups work.
- Billing. Check it supports the funders and insurers you work with.
- Export. Make sure you can take your records with you if you leave.
Solo OTs and growing clinics
A solo OT usually needs online booking, reminders, notes and invoicing that take minutes to set up. As a clinic grows, the priorities shift to shared calendars, team permissions, reporting and billing that can handle several funders at once.
It's worth choosing with the next two years in mind. Moving records between systems is slow, so a tool that suits a team of five is often a better buy than one you'll outgrow quickly.
A sister therapy practice has its own tools, compared in a roundup of speech therapy software.
A simple switching plan
- Map your current steps from referral to discharge.
- Trial two systems with real sessions for a few weeks.
- Build your templates before you move any records.
- Train the whole team on one routine at the same time.
Do you make occupational therapy, rehab or practice management software? Tool makers can ask to be considered for a featured place on this list.
My plain advice on occupational therapy software
Choose a practice system that fits your setting, add an AI scribe only once your templates are right, and test everything with real sessions before committing. The best tool is the one that gets your notes finished before you leave the clinic.
If clients still book by phone, my guide on how to set up appointment booking on your website is a quick win alongside new software.
